A steady bridge between hospital and a home of your own

LifeBridge Supported Living Ltd supports adults aged 18 to 65 living with mental health conditions. That includes psychosis, bipolar disorder and dual diagnosis. We help people move on from hospital and build a settled, independent life in their own tenancy.

Who we are, and who we are not

LifeBridge Supported Living Ltd (trading as LifeBridge) is a UK supported living provider for working-age adults with mental health needs. We are not a hospital, a nursing home, a children's service or a domiciliary care agency for older people, and we are not connected to other organisations that use the LifeBridge name. Everything we do is built around one transition: from a ward or a placement that no longer fits, to a home and a life a person chooses.

Your own tenancy, real support

People hold their own tenancy and their own front door. Support is arranged around the person, not around a building's routine.

Trauma-informed, not clinical

We start from what has happened to someone, not what is wrong with them. Relationships come first, paperwork second.

Planned step-down

Support hours flex down as confidence grows, with clear, evidenced outcomes shared with commissioners and clinical teams.

For families

If someone you love is being discharged from hospital, the questions are usually simple and urgent: where will they live, who will be there, and what happens if things get hard again? We will talk you through all of it in plain language, and we will tell you honestly if LifeBridge is not the right fit.

For commissioners and discharge teams

We accept referrals from local authority commissioners, NHS discharge and community mental health teams, and social workers. Send us the essentials and we will respond with a clear yes, no, or what else we would need. No drawn-out silence while a bed is held.
